Collect a sample of the\u00a0<strong><em>first\u00a0<\/em><\/strong>bowel movement of the day.<br \/>\n3. Collect samples\u00a0<strong><em>only\u00a0<\/em><\/strong>from Sunday to Wednesday.<br \/>\n4. Mail samples\u00a0<strong>only<\/strong>from Monday to Thursday<\/li>\n<\/ol>\n<p><strong>Kit contents for all participants except San Francisco<\/strong><\/p>\n<p>2 copies of the informed consent<br \/>\n1 set of instructions for completing food frequency questionnaire (FFQ) online<br \/>\n1 hard copy of FFQ<br \/>\n2 Paper sample collectors (toilet hats)<br \/>\n2 Alcohol wipes<br \/>\n2 Brown screw-cap sample collection tubes<br \/>\n2 Red cap sterile qtip collection tubes<br \/>\n2 Biosafety bags (for samples)<br \/>\n4 Ice packs<br \/>\n2 Silver padded envelopes<br \/>\n1 Ziploc bag (if needed, for consent)<br \/>\n2 Pre-paid flat-rate padded shipping envelopes<\/p>\n<p><strong>Sample collection for all participants except San Francisco<\/strong><\/p>\n<ol>\n<li>Please write the <strong><em>date of sample collection <\/em><\/strong>and<strong><em> your initials <\/em><\/strong>on the label of the sample collection tubes.<br \/>\n2. Stick the paper sample collector (hat) onto toilet seat according to instructions printed on paper collection device (<strong><em>do not urinate on collection device<\/em><\/strong>).<br \/>\n3. Following bowel movement, unscrew the lid of the brown-cap sample collection tube \u2013 the lid has a scoop attached, using the scoop, collect a <strong><em>small <\/em><\/strong>scoopful of stool from the collection hat.<br \/>\n4. Re-screw the lid tightly on the sample collection tube.<br \/>\n5. Peel open the qtips container and remove the (red) cap.<br \/>\n6. Rub the cotton end of both qtips against the used toilet paper so as to stain the qtips brown. No solid material should remain in the qtips (if they stain brown, it is plenty already!)<br \/>\n7. Remaining stool and paper collection device may be flushed down the toilet (see instructions on collection device).<br \/>\n8. Place both collection tubes (brown-cap and red-cap) in provided ziploc bag<br \/>\n9. Wash your hands with soap and warm water for at least 30 seconds.<\/li>\n<\/ol>\n<p><strong>Packing and shipping\u00a0of sample for all participants except San Francisco<\/strong><\/p>\n<ol start=\"10\">\n<li>Place the completed and signed consent form (only if not signed in person) into the second (larger) ziploc bag, seal the bag.<br \/>\n11. Place\u00a0 the\u00a0 ziploc\u00a0 bag\u00a0 containing\u00a0 the\u00a0 stool\u00a0 collection\u00a0 tubes\u00a0 inside\u00a0 the\u00a0 silver\u00a0 padded envelope.<br \/>\n12. Place two frozen ice packs in the silver padded envelope.<br \/>\n13. Place the silver padded envelope inside the pre-paid flat-rate shipping envelope<br \/>\n14. Mail samples\u00a0<strong>only<\/strong>from Monday to Thursday<\/li>\n<\/ol>\n<p><strong>Kit contents for San Francisco participants ONLY<\/strong><\/p>\n<p>2 Paper sample collectors (toilet hats)<br \/>\n2\u00a0Pairs of gloves<br \/>\n2 <strong>Empty <\/strong>Brown screw-cap sample collection tubes<br \/>\n2 Brown screw-cap sample collection tubes <strong>filled with liquid<\/strong> (culture medium)<br \/>\n2 Red cap sterile q-tip collection tubes<br \/>\n2 Black Ziploc bags (for samples)<br \/>\n2 Ice packs (1 per sample shipment)<br \/>\n2 White padded mailers<br \/>\n2\u00a0 Shipping envelopes<\/p>\n<p><strong>Sample collection for San Francisco participants ONLY<\/strong><\/p>\n<p>You will find three different tubes per sample collection \u2013 1 empty brown cap, 1 brown cap with liquid, and 1 red cap with a q-tip swab.<br \/>\n<strong>1. <\/strong>Please write the <strong><em>date of sample collection <\/em><\/strong>and<strong><em> your initials <\/em><\/strong>on the label of all the sample collection tubes.<br \/>\n<strong>2. <\/strong>Place the paper sample collector (hat) onto the toilet seat according to instructions printed on paper collection device (<strong><em>do not urinate on collection device<\/em><\/strong>).<br \/>\n<strong>3. <\/strong>Following a bowel movement, unscrew the lid of the <strong>empty brown-cap sample collection tube<\/strong> \u2013 the lid has a scoop attached, using the scoop, collect a <strong><em>small <\/em><\/strong>scoopful of stool (the size of a pea is sufficient) from the collection hat.<br \/>\n<strong>4. <\/strong>Re-screw the lid tightly on the sample collection tube.<br \/>\n<strong>5.<\/strong> Unscrew the lid of the sample collection tube <strong>(brown cap) filled with liquid<\/strong> using care not to spill the liquid inside (culture medium). The tube lid has a scoop attached; using the scoop, collect a <strong><em>small <\/em><\/strong>scoopful of stool from the collection hat into the tube. Use caution and try to minimize the time that the collection tube remains open.<br \/>\n<strong>6.<\/strong> Re-screw the lid tightly on the sample collection tube.<br \/>\n<strong>7.<\/strong> Shake the tube vigorously for about 10 seconds to ensure dispersion of the sample within the solution (culture medium).<br \/>\n<strong>8. <\/strong>Peel open the q-tips sleeve and remove the (red) cap. Do not discard the plastic sleeve.<br \/>\n<strong>9. <\/strong>Rub the cotton end of both q-tips against the used toilet paper so as to stain the q-tips brown. No solid material should remain in the q-tips (if they stain brown, it is plenty already!)<br \/>\n<strong>10. <\/strong>Remaining stool and paper collection device may be flushed down the toilet (see instructions on collection device).<br \/>\n<strong>11.<\/strong> Individually package each of the collection tubes (brown caps) in the provided containers, and put the q-tip swab back into its plastic sleeve.<br \/>\n<strong>12.<\/strong> Place the contained sample collection tubes (2 brown-cap and 1 red-cap per sample) in the biosafety bag (make sure to remove excess air within the bag). The bag should contain 2 brown cap tubes in their respective containers as well as the q-tip swab in the plastic sleeve.<br \/>\n<strong>13.<\/strong> Wash your hands with soap and warm water for at least 30 seconds.<br \/>\n<strong>14.<\/strong> Place the biosafety bag containing the samples in the freezer <strong>overnight<\/strong>.<\/p>\n<p><strong>Packing and shipping\u00a0of sample for San Francisco participants ONLY<\/strong><\/p>\n<ol start=\"15\">\n<li>Place the Ziploc bag containing the <strong>frozen <\/strong>stool collection tubes inside the white padded envelope.<br \/>\n<strong>16.<\/strong> Place <strong>one<\/strong> of the frozen ice packs in the white padded envelope. Make sure the ice pack touches the samples inside the padded envelope.<br \/>\nTo Seal the padded envelope, you will see two adhesive peels labeled, \u201cREMOVE TO EXPOSE ADHESIVE.\u201d One is located on the <strong>inside <\/strong>of the padded mailer near the top, the other is located on the <strong>envelope flap<\/strong>.<br \/>\n<strong>17.<\/strong> First, remove the adhesive peel on the <strong>inside<\/strong> of the padded portion and seal the envelope<br \/>\nshut such that you cannot see the samples inside.<br \/>\n<strong>18<\/strong>. Then, remove the adhesive peel at the <strong>flap<\/strong> of the envelope to seal the entire mailer shut.<br \/>\n<strong>19.<\/strong> Place the padded envelope inside the pre-paid shipping envelope and seal the shipping envelope.<br \/>\n<strong>20.<\/strong> Mail the pre-paid envelope, by dropping it off at the post office, as soon as possible to ensure overnight delivery to our lab.<\/li>\n<\/ol>\n<p><strong><em>Please remember: In order to avoid sample delivery on a weekend, please only mail samples from Sunday through Thursday.<\/em><\/strong><\/p>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>FOR ALL PARTICIPANTS EXCEPT SAN FRANCISCO Instructions for participants in\u00a0San Francisco\u00a0are at the\u00a0end of this page.
