Microbiome Online Food Questionnaire Instructions<\/h1>\n

To complete\u00a0the\u00a0food survey, copy the following Internet address (URL)\u00a0into your internet browser and login with the user login name, password and group ID provided to you.\u00a0It should take about 30 to 40 minutes to complete the survey. If you need to leave the survey session before you have completed it, you can login again later and complete it (see Item 6 below for instructions).<\/p>\n

Features of the online system that are useful\u00a0to know:<\/p>\n

  1. Login information is case-sensitive.<\/li>\n
  2. Each food\/supplement question comes with a “More Info”<\/em>\u00a0link. Click\u00a0this button for help in understanding this\u00a0food\/supplement question.<\/li>\n
  3. Click on the “Next” <\/em>button to proceed from one screen to the next. Each time you click the “Next”<\/em> button, your answers are recorded by the online questionnaire system.<\/li>\n
  4. Be sure to answer\u00a0all questions that appear on each\u00a0screen so that you can go on to the next screen.\u00a0A red check mark will appear to the left of each question completed.<\/li>\n
  5. You can change\u00a0answers easily.\u00a0Just\u00a0use the “Back” <\/em>button\u00a0at the\u00a0top of the questionnaire page to go back to an earlier screen (Do\u00a0Not<\/u>\u00a0use the\u00a0“back” arrow on your internet browser).<\/li>\n
  6. If you need to stop the survey session before completion, click the “Stop Survey” <\/em>button in the upper right hand corner of the questionnaire screen. You can easily resume the questionnaire session by logging in again later.\u00a0The system is “smart” enough\u00a0to take you\u00a0directly to the last screen you visited before stopping.<\/li>\n
  7. Once you have completed the final section of the survey, click the “Next” <\/em>button again to be sure that all your answers are\u00a0transmitted to the online system.<\/li>\n<\/ol>\n
